I will no longer be seeing patients at Family Medicine Clinic at 8738 North Lamar Blvd, Austin TX 78753 after January 31st, 2022.  The physician assistant, Erin Rehberg will also not be available after this date.

You may obtain copies of your medical records or have them transferred through the staff here at the clinic. You may also fill out a form at the front desk or submit a request by mail or fax to:
